Accessing the Form Builder
You can open the builder in two places:- Inside a project — go to your project, open the Forms tab, and click Add Form. Forms created here belong to that project.
- Globally — from the main sidebar, go to Forms → New Form. Useful when you’re building a form that isn’t tied to a specific project yet, or when you want to reuse the structure across projects.
Structuring the Form With Sections
Every form is organized into Sections — logical groupings that hold your fields. Section headers give your field officers context as they move through longer forms and help them keep their place without losing track of what data they’ve already captured. Typical sections in a beneficiary intake form might be Personal Info, Household Data, and Health History. Add a section from the builder’s left panel, name it, and drag it onto the canvas. Fields you add next will sit inside the currently selected section. For very short forms (five fields or fewer) a single default section is fine. Anything longer benefits from grouping.Adding Fields
1
Drag a field type onto the canvas
Choose a field type from the left panel and drag it into a section. You can reorder fields at any time by dragging them up or down, and move them between sections the same way.
2
Configure the field
Click any field on the canvas to open its settings panel on the right. Set the field Label (what your field officers see), an optional Placeholder (hint text inside the input), and Help text (a short instruction shown beneath the field), then toggle Required or Optional.
3
Set validations where needed
For text and number fields, you can enforce min/max length, min/max value, or a regex pattern to catch malformed values at the point of collection rather than during report generation.
4
Tag sensitive fields
For fields that may contain personal data — names, phone numbers, national IDs, GPS coordinates — open the field settings and set the Sensitivity tag to Personally Identifiable or Restricted. See Tagging Sensitive Fields below for why this matters.
5
Repeat for all fields
Continue adding, configuring, and grouping fields until your form covers everything your programme needs to capture.
Field Types Available
Terriqon supports the following field types:Tagging Sensitive Fields
When you tag a field as Personally Identifiable or Restricted, Terriqon excludes that field from the dataset the AI receives during report generation. The AI works only on aggregated, anonymised metrics — it never sees the raw values of tagged fields. Because exclusion is driven by your tags, tagging every PII field when you build the form is what keeps personal data out of the pipeline. Two important consequences follow:- Beneficiary privacy is protected. Names, IDs, phone numbers, and location data you’ve tagged never enter the AI processing layer.
- Data governance is supported. Your reports are generated from aggregated metrics, not from identifiable records — supporting your GDPR obligations and most donor data governance requirements. Terriqon strictly follows GDPR principles, though it does not hold a formal certification.
Linking a Form to a Registry Type
If your form is registering or updating an entity you’re tracking over time — a beneficiary, a facility, a school, an asset — link it to the appropriate Registry Type so approved submissions create or update the right record automatically. From the Registry Types list, open the registry you want and link this form. From that point, each approved submission is matched against existing records by the registry’s Primary Identifier, and either creates a new record or updates the matching one. You don’t need to link a form to a registry. Standalone forms (surveys, monitoring checklists, one-off data collection) work perfectly well — submissions are simply reviewed and stored without updating any entity. See Forms for the difference between the two paths.Assigning Field Officers
Every form needs at least one Field Officer assigned before it will appear on any device. From the form’s settings, specify which officers can submit this form from the mobile app. Only the officers you assign will see the form on their home screen. You can add or remove assigned officers at any time. Changes take effect the next time each device syncs.Publishing
Before you publish, click Preview in the top toolbar. The preview renders your form exactly as it will appear on a mobile device, so you can check field order, label clarity, section grouping, and layout on a small screen. When you’re satisfied with the form:1
Click Publish
The form moves from Draft to Published status.
2
The form appears on assigned devices
Assigned Field Officers see the form on their home screen the next time their device syncs. If they’re offline at publish time, they’ll receive it as soon as connectivity returns.
Unpublishing is useful when you need to pause a form (for example, mid-cycle changes to a monitoring schedule) without losing the data already collected.
